You must complete and return all athletics forms and Health & Wellness Center forms prior to participation in intercollegiate athletics.

ATHLETICS FORMS
In order to participate in intercollegiate athletics at Misericordia University, it is mandated that each student-athlete complete the forms listed below.   Completed forms are to be returned to Misericordia University Athletic Department, 301 Lake Street, Dallas, PA  18612.  If you have any questions, please call Robin Jackson, ATC, at 570-674-6349.
 
 
1.       Physical Examination Form (PDF) (exam must be dated within 6 months of your first intercollegiate practice).
2.       Insurance Information Form (DOCX)
3.       Proof of current insurance (provide a copy of the front and back of insurance card).
4.       Sickle Cell - Student-athletes must provide one of the following - this is a one-time requirement for any new student-athlete at Misericordia.
            A. Proof of prior testing;
            B. Obtain a sickle cell test and provide the results.

HEALTH & WELLNESS CENTER FORMS

You must be logged into the mymu portal:

All new/transfer students-  Log into mymu portal, click on the orientation tab under Student Life to access Medicat to complete all mandated Health & Wellness Center school requirements:  https://mymu.misericordia.edu/ICS/ORIENTATION/Step_3.jnz. 

Returning student-athletes-  Log into mymu portal, click on the health and wellness tab under Student Life to update/complete all Medicat requirements:  

https://mymu.misericordia.edu/ICS/Campus_Life/Health_and_Wellness/MEDICAT.jnz.

These forms must be updated annually.  If you have any changes to your insurance, you must provide a copy of the front and back of your new insurance card to both the Athletic Department and Health & Wellness Center immediately.
